The aim of the study was to examine the association between neonatal hospitalization and risk of cerebral palsy. We conducted a case-control study of children born in 1993-1998 in the United States and receiving services in the postneonatal period through 2005. We obtained state birth certificate data for the children and state hospital discharge data. Cases were children diagnosed with cerebral palsy (n = 993) and, as controls, 2:1 age- and sex-matched, randomly selected, children diagnosed with other noncerebral developmental disorders or who had not received specialty neonatal care and whose state hospital discharge records indicated that they were normal at birth (n = 3044). We used conditional logistic regression to obtain crude and adjusted odds ratios (ORs) and 95% confidence intervals (CIs). Independent variables included neonatal hospitalization, birth year, ethnicity, mothers' age at delivery, mothers' marital status, mothers' education, mothers' employment status, and state of residence. We also conducted analysis by type of cerebral palsy (spastic and non-spastic). Neonatal hospitalization was associated with increased risk of cerebral palsy (adjusted OR = 1.49, 95% CI: 1.28, 1.75). There was a modest but statistically significant association of risk with increasing birth year. There was no statistically significant association with race or ethnicity, mothers' age at delivery, marital status, education, or employment. Analysis by type of cerebral palsy revealed increased risk for both spastic (adjusted OR = 1.44, 95% CI: 1.11, 1.89) and non-spastic (adjusted OR = 1.59, 95% CI: 1.15, 2.20) cerebral palsy. Neonatal hospitalization was associated with increased risk of cerebral palsy. There was no association with any birth characteristics that could be used to guide targeted interventions to prevent cerebral palsy.
